Disclaimer and Informed Consent Agreement

Reiki and Sound Healing are energy-based techniques that aims to promote stress reduction and relaxation. It operates on the principle that the human body possesses an intrinsic ability to balance and heal itself, a process which can often be aided through achieving a state of deep relaxation. Please note that achieving balance in cases of long-standing imbalances may require multiple Reiki sessions to achieve the optimal level of relaxation for self-healing.

By opting for alternative services, you acknowledge and understand the following:

• Holistic Approach: Reiki sessions are provided as a holistic approach to well-being. The use of Reiki services should be seen as a complement to, rather than a substitute for, traditional medical approaches. Reiki sessions are not intended to replace physical or mental clinical diagnosis, treatment, cure, or professional      therapy. If you require medical attention, always consult your physician or other qualified health provider for advice regarding your medical condition.
• Non-Medical Practitioners: Our Reiki practitioners are not medical professionals. Therefore:
  • They do not diagnose medical conditions.
  • They do not prescribe or administer any form of medical treatment or       substances.
  • They do not provide any medical advice.
  • They do not perform medical treatment, nor do they interfere with the       treatment prescribed by a medical professional.
• Potential Benefits and Risks: While Reiki sessions may provide potential benefits such as relaxation, stress reduction, and a sense of well-being, it’s important to recognize that      individual experiences and outcomes may vary.
• Confidentiality: Any personal information or discussions shared during the Reiki sessions will be treated with utmost confidentiality. Your privacy will be respected, and no information will be disclosed to third parties without your explicit      consent, except as required by law.
• Voluntary Participation: You affirm that your participation in Reiki sessions is entirely voluntary and that you have chosen to engage in this practice of your own free will. You retain the right to discontinue the session at any time if you feel      uncomfortable or overwhelmed.
• Informed Decision: You have been given the opportunity to ask questions and seek clarification regarding the nature, purpose, potential benefits, and risks associated with Reiki.  With this knowledge, you have made an informed decision to participate in the Reiki sessions.
• Disclaimer of Liability: By booking an appointment you agree to this disclaimer, you assume full responsibility for your own health and well-being. You understand that our Reiki services are provided without any guarantees or warranties, and you release us from any liability arising from the use of our Reiki services.  You acknowledge that any claims or causes of action, known or unknown, are      waived.

In consideration of the above, you voluntarily agree to participate in Reiki sessions, assuming all risks associated with the practice and releasing us from any liability.

If you are experiencing a physical or psychological ailment, it is highly recommended that you seek the attention and advice of a licensed physician or qualified healthcare professional.

Release of Liability

In consideration of the risk of injury while participating in Light and Sound Waves LLC services (the “Activity”), and as consideration for the right to participate in the Activity, I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation in the Activity, and do hereby release and forever discharge Light and Sound Waves LLC, located at 1953 South 1100 East, Suite 522013, Salt Lake City, UT 84106, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any physical or psychological injury, including but not limited to illness, paralysis, death, damages, economical or emotional loss, that I may suffer as a direct result of my participation in the aforementioned Activity, including traveling to and from an event related to this Activity.

I am voluntarily participating in the aforementioned activity and I am participating in the activity entirely at my own risk. I am aware of the risks associated with traveling to and from as well as participating in this activity, which may include, but are not limited to, physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), economic or emotional loss, and death. I understand that these injuries or outcomes may arise from my own or others’ negligence, conditions related to travel, or the condition of the activity location(s). Nonetheless, I assume all related risks, both known or unknown to me, of my participation in this activity, including travel to, from and during this activity.

I agree to indemnify and hold harmless Light and Sound Waves LLC against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney’s fees and any related costs, if litigation arises pursuant to any claims made by me or by anyone else acting on my behalf. If Light and Sound Waves LLC incurs any of these types of expenses, I agree to reimburse Light and Sound Waves LLC.

I acknowledge that Light and Sound Waves LLC and their directors, officers, volunteers, representatives and agents are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of Light and Sound Waves LLC.

I acknowledge that this activity may involve a test of a person’s physical and mental limits and may carry with it the potential for death, serious injury, and property loss. The risks may include, but are not limited to, those caused by terrain, facilities, temperature, weather, lack of hydration, condition of participants, equipment, vehicular traffic and actions of others, including but not limited to, participants, volunteers, spectators, coaches, event officials and event monitors, and/or producers of the event.

I acknowledge that I have carefully read this “waiver and release” and fully understand that it is a release of liability. I expressly agree to release and discharge Light and Sound Waves LLC and all of its affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I otherwise have to bring a legal action against Light and Sound Waves LLC for personal injury or property damage.

To the extent that statute or case law does not prohibit releases for negligence, this release is also for negligence on the part of Light and Sound Waves LLC, its agents, and employees.

If I should require medical care or treatment, I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance.

If any damage to equipment or facilities occurs as a result of my or my family’s willful actions, neglect or recklessness, I acknowledge and agree to be held liable for any and all costs associated with any actions of neglect or recklessness.

By booking an appointment and have agreed to this Disclaimer, Informed Consent Agreement, and Release of Liability you confirm that you have read, understood, and agreed to all terms and conditions outlined herein.
